I hereby grant Sylvan Veterinary Hospital permission to use my name and likeness in photograph(s)/video in any and all of its publications and in any and all other media, whether now known or hereafter existing, controlled by Sylvan Veterinary Hospital, in perpetuity, and for other use by Sylvan Veterinary Hospital. I will make no monetary or other claim against Sylvan Veterinary Hospital for the use of photograph(s)/video.
